Vitamin B12 (Cobalamin): Energy, Hair and Why the Route Matters

Vitamin B12 is the least glamorous ingredient in our IV lounge and, for a large share of Jaipur clients, the one most likely to be genuinely missing. It is not a skin-brightening active and we will not pretend it is. It is the vitamin that runs your red blood cells, your nerves and the fast-dividing cells of your hair follicles — and India has one of the highest rates of B12 deficiency in the world, concentrated in exactly the vegetarian, milk-light diets common in Rajasthan. This page explains what B12 does, why tablets so often fail to fix a low level, how we use it in energy and hair drips, and the side effect that surprises people.

What B12 does in the body

Cobalamin is a large, complex molecule built around a single cobalt atom — the only vitamin that contains a metal. It is a cofactor for two enzymes, and both matter for how you look and feel.

  • Methionine synthase, which recycles homocysteine into methionine and keeps the folate cycle turning. Without it, cells cannot make DNA efficiently — and the cells that divide fastest suffer first: the bone marrow that makes red blood cells, the gut lining, and the hair follicle matrix.
  • Methylmalonyl-CoA mutase, part of energy metabolism in mitochondria and essential for maintaining the myelin sheath around nerves. This is why B12 deficiency shows up as fatigue, pins and needles, poor concentration and low mood before anyone thinks of a vitamin.

Why Jaipur in particular

Studies across India consistently find low B12 in a large share of healthy adults, and the pattern is strongest in vegetarian populations with modest dairy intake. Rajasthan’s food culture — largely vegetarian, with milk that is often boiled or made into products that lose some B12 — sits squarely in that pattern. Add three common medicines that block absorption, metformin for diabetes and the acid-reducing tablets taken for reflux, and a hot climate where fatigue is written off as “the weather”, and you have a city where a surprising number of people walk around mildly deficient without knowing it.

For hair, this matters directly. Hair follicle cells divide as fast as any in the body; when B12 (or iron, or vitamin D) is short, hair shifts into its resting phase and sheds a few months later — the diffuse thinning that clients describe as “my hair is falling from everywhere”. Correcting the deficiency is not a cosmetic hair treatment; it removes a cause. That is why our hair-fall screening asks about diet and medicines before anyone talks about PRP.

Why tablets often don’t fix it — and when IV makes sense

Absorbing B12 from food or tablets is an elaborate process: stomach acid frees it, a protein called intrinsic factor binds it, and a specific receptor in the last part of the small intestine takes it in — with a hard ceiling of a few micrograms per meal. Anyone with low stomach acid, long-term acid-reducing tablets, metformin, gut conditions or simply the natural decline in intrinsic factor with age absorbs far less. High-dose oral B12 works around this through a small passive leak, which is why 1,000–2,000 µg tablets can raise levels — slowly, over months, if taken consistently.

Giving B12 intravenously or by injection bypasses the whole chain. Blood levels rise immediately and the liver stores fill quickly. In our IV lounge, B12 is used where that speed matters: a client with symptomatic fatigue and a low or borderline level, a hair-fall programme where correcting the deficiency early shortens the wait for regrowth, or recovery drips where energy metabolism is the goal. It is not used as a routine “energy shot” for someone whose level is fine — extra B12 above need is simply excreted.

The form we use is chosen for the goal — methylcobalamin for its direct coenzyme activity in most wellness drips — and the dose is set by the screening and reviewed by the doctor. If your screening suggests a real deficiency, we will also recommend a blood test and a longer-term plan, because a drip is a correction, not a diet.

The side effect nobody mentions

High-dose B12 can cause acne. It is well documented: a sudden crop of small, uniform red bumps across the face, chest and back a week or two after large B12 doses, which clears when the dose stops. The mechanism appears to involve B12 altering the metabolism of the skin’s acne bacteria towards more inflammatory by-products. It is uncommon, more likely in people already prone to breakouts, and it is the reason our screening asks about acne before adding B12 to a drip — and why a client on an acne plan is given B12 only if a deficiency actually needs correcting.

Who should not have IV B12

  • Cobalt or cobalamin allergy, which is rare but real
  • Leber’s hereditary optic neuropathy — cyanocobalamin specifically is avoided; other forms are discussed with your physician
  • Pregnancy and breastfeeding — IV wellness drips are deferred; dietary and prescribed B12 are a matter for your obstetrician
  • Active acne flare, unless a deficiency is confirmed and the trade-off is discussed
  • Active infection or fever on the day
  • Anyone whose symptoms suggest a cause that needs a diagnosis first — unexplained severe fatigue, neurological symptoms or anaemia are investigated, not dripped

What to expect

B12 is one component of a drip that takes 45 to 60 minutes; it adds nothing to the sensation. If you were deficient, the change is real and often noticeable within days — energy, concentration, fewer pins-and-needles — and hair shedding slows over the following two to three months as new growth cycles begin. If you were not deficient, you will not feel much, and we will have told you that in advance. Results vary, and any hair benefit depends on the rest of the plan: iron and vitamin D status, thyroid, hormones and scalp health.

Why are so many vegetarians low in B12?

Can B12 cause acne?

High doses can trigger an acneiform eruption in some people, especially those already prone to breakouts. We ask about acne before including B12 and weigh it against any deficiency.

Does B12 help skin glow?

Not directly. It is not a brightening or antioxidant active. Correcting a deficiency can improve the pallor and dullness that come with it, but for skin tone our vitamin C and glutathione pages are the relevant ones.

Should I get my B12 tested first?

If you have symptoms, are vegetarian, take metformin or acid reducers, or are on a hair-fall plan — yes. A serum B12 test is inexpensive and lets us plan a correction rather than guess.

How quickly does IV B12 work?
